Home Health experience is strongly preferred. Home Care Home Base experience is preferred.

:

The Home Health Intake Coordinator Full Time is responsible for providing a high level of customer service to our patients and their caregivers from intake to discharge for our Home Health office.

Schedule:

Weekend – Full Time

Friday/Saturday/Sunday/Monday

4 – 10 hour days

Training period will be Monday - Fridays, 9a-4p. Once complete, you would transition to the Weekend hours and possible to go Remote. Training in Bradenton area and not offering reimbursement for travel there or overnight.

Essential Job Functions may include:

  • Contacts patients to: advise them of the order placed on their behalf and to confirm all patient demographic information; to communicate any financial responsibilities and/or additional information necessary for processing the order received
  • Scheduling Discipline visits and collaborating with staff providing patient care
  • Communicating with community Healthcare providers to assist in patient care
  • Demonstrates professional etiquette and courtesy when interfacing with customers
  • Complies with and adheres to all regulatory compliance areas, policies and procedures

Requirements:

  • High school diploma or GED required/LPN preferred
  • Minimum of 2 years related experience
  • Experience with HCHB a plus
  • Proficient in Microsoft suite of products including Outlook, Word and Excel
  • Proven ability to manage in a high call volume environment with accuracy and efficiency
  • Demonstrated ability to prioritize multiple tasks to meet deadlines
  • Demonstrated ability to interact in a collaborative manner with other departments and teams
  • Medical office, customer service preferred
